Spinal cord ependymoma presenting as acute paraplegia and subarachnoid hemorrhage: a case report and review of literature

چکیده
Acute paraplegia is a rare presentation for a spinal cord ependymoma. Subarachnoid hemorrhage (SAH) due to spinal ependymoma is also very rare. We report a 32-year-old woman who presented with acute paraplegia and typical clinical signs of SAH with normal cerebral angiography, and further diagnostic work-up revealed an spinal cord ependymoma as the source of the hemorrhage. There is evidence that some spinal cord ependymomas have intratumoral hemorrhage, but most of these bleedings occur without symptoms. We discuss the clinical and neuroradiological findings of this rare case and review the literature related to this unusual presentation. There is evidence both from surgery and magnetic resonance imaging (MRI) that some of the spinal ependymoma have intratumoral bleeding. However, most of these bleedings occur without symptoms. Based on Pubmed, there were only two cases of the spinal cord ependymoma presenting with acute paraplegia due to intratumoral and subarachnoid hemorrhage being reported since 1990. In the series by McCormick et al., of the 23 patients with ependymoma of the filum terminale, only two suffered acute neurological deterioration. In these two cases, MRI was not performed and intratumoral hemorrhage was confirmed at surgery. Subarachnoid haemorrhage (SAH) due to spinal ependymoma is also very rare. A review of the literature based on Pubmed showed that only 17 patients with spontaneous SAH due to a spinal ependymoma had been reported since 1958. We describe here a case of spontaneous intratumoral bleeding from a spinal ependymoma presented with acute paraplegia and SAH with normal cerebral angiography.

Acute Paraplegia as a Result of Hemorrhagic Spinal Ependymoma Masked by Spinal Anesthesia: Case Report and Review of Literature.
Ependymomas are the most common intramedullary spinal cord tumors in adults. Although a hemorrhage within spinal ependymoma on imaging studies is not uncommon, it has rarely been reported to bea cause of acute neurological deficit. In the present report, we describe a case of a 24-year-old female patient who developed acute paraplegia as a result of hemorrhagic spinal ependymoma immediately aft...
متن کاملSPINAL SUBDURAL HAEMATOMA WITH PARAPLEGIA MIMICKING DORSOLUMBAR PROLAPSED DISC- A RARE CASE REPORT
Background- Spinal subdural haematoma (SSH) is a rare condition with unknown incidence in the general population. Iatrogenic spinal subdural haematoma radiologically mimicking a prolapsed dorso-lumbar disc has not been published in the literature. Case presentation- A 65-yr-old female presented with altered sensorium and generalized weakness for 3 days was evaluated by Neurologist and diagn...
متن کاملParaplegia due to Acute Aortic Coarctation and Occlusion
Coarctation and occlusion of the aorta is a rare condition that typically presents with hypertension or cardiac failure. However, neuropathy or myelopathy may be the presenting features of the condition when an intraspinal subarachnoid hemorrhage has compressed the spinal cord causing ischemia. We report two cases of middle-aged males who developed acute non-traumatic paraplegia. Undiagnosed co...
متن کاملClear Cell Ependymoma of Spinal Cord: A Case Report
Clear cell variant of ependymoma is almost exclusively located in the supratentorial region. Only few cases of this tumor that located in the spinal cord have been reported. Here we report one case of intramedullary clear cell ependymoma of the lumbar spinal cord. In microscopic examination, the tumor cells were round to oval with moderate amounts of clear cytoplasm and centrally located lar...
متن کاملSpinal ependymoma presenting as subarachnoid hemorrhage and leading to superficial siderosis of the central nervous system: a case report
Superficial siderosis of the central nervous system is a very uncommon disorder. We report a rare case of spinal ependymoma in a 58-year-old man whose main complaints at presentation were recent lumbago and headache, and he was evaluated to confirm suspected spinal subarachnoid hemorrhage (SAH). After cerebrospinal fluid analysis and imaging studies the patient was diagnosed with SAH and superf...
